There is a major plot twist in which the main character is revealed to be part of a system, and she is totally unaware to this fact. On one hand, the execution does not lean heavily in the direction of "insane serial killer" and the alters are treated respectfully after the twist is revealed. On the other hand, this twist reveals that the character has been consistently hallucinating the alters and other things over the course of the game. I will not say that the topic was handled in a delicate manner, but there are very genuine criticisms to be had with the depiction.
