we only see Ralph make human sacrifices, i can't recall animals ever being involved in any violence in the game. sometimes a choice you can make as one way to artificially get closer to your neighbor, you can "return" their escaped pet, implying you're the one who abducted it just to lie about having found it outside and claiming to just be returning it. however, there's no implication of physical harm to the animal in the process. i think we can assume nothing more happens to the pet than some confusion.

one of the game's main mechanics is a ritual sacrifice you're made to commit where you mutilate the victim's organs in very specific ways. not always an easy game for the squeamish, but i guess you can skip those scenes easily enough. there are bits of gore in many of the cutscenes tho, to varying degrees of graphic-ness

someone is kept in a cage and regularly fed and presumably cleaned but physically left to atrophy in the dark most of the day for 10 years, as well as incidentally being forced annually to witness a brutal ritual sacrifice. not typical domestic violence, but it does happen in a suburban home and themes of abusive relationships in general are present in the overall story so it could be uncomfortable

both endings are incredibly tragic. one of the points of this game is the unavoidable weight of the protagonist's actions, whether or not they have good intentions behind them. Ralph is never able to be absolved of his gruesome actions, and Johnny is never able to get back what was taken from him whether or not he escapes.

the game is definitely about breaking cycles of abuse in a metaphorical sense, but the exact dynamics of abuse between 'all-powerful god and his victim who believes he has no possible way to escape without harming many others in the process' and 'man keeping another man in his basement specifically to protect the secrecy of the works he's forced into by said god' are like... not exactly equivocal. though Ralph is not at all excused because we're not sure if the god supposedly forcing him to do all this is even real, i think it is much different than a person abused by another person who then goes on to later abuse a different person in the same way they were. i hope this makes sense lol

Ralph says he has to refer to people as "beasts" for slaughter to Zimforth as a coping mechanism, because viewing them as real people is too daunting for him. he feels tremendous guilt for the things he feels forced to do, so he tells himself he's not even a real person in order to shield himself enough emotionally that he can continue to do Zimforth's violent biddings, to the point where every time you see and hear his name on the screen, it's distorted. because he can't accept himself as a real autonomous person.

for a game that causes you to grapple with how much of the protagonist's experience is real or not, it doesn't actually address existence itself very much. it's clear that whether or not Zimforth and his demands for Ralph are real or hallucinatory, the dirty deeds he carries out for Zimforth and his own guilt in doing so are very very real.
