Scratch can die on a rare occasion. Just don’t attack him in camp or give him back to the kennel owner at the end of the game - as she is said to be abusing the dogs. You can stop her and send her packing.
A witch named Auntie Ethel can trade your eye by plucking it out And another character name Volo can try and use a pick to try and get the mind flayer worm out of your head by stabbing your eye
There’s a cutscene at the start of the game where the mind flayer worm crawls into A character called Lae’zel’s eye and then one gets put into yours via POV It’s not too gruesome but not pleasant still
It doesn’t show gore but the idea isn’t very nice still
There are a few instances where you as the main character can kill children - this is obviously your choice to do so
Sometimes in Act 3 a character named Orion can kill a child who will seek shelter in your camp named Yenna. This is played for shock but can be quite gruesome

There are several moments in the game that shows blood, guts and gore, especially with a character named Orin who is a vicious murderer. There are various moments where there are close ups of bloodied dead bodies which may be upsetting for some people
In Act 3 - there are some Gallows Immediately after Wyrms Crossing right at the start of Basilisk Gate. You can avoid it by not interacting and running past it with the camera moved.
Sometimes there is a side quest involving a character called Florrick and if you don’t save her, her body will be hung at the gallows and stay there for the remainder of the game.
Many of the “Evil” Endings for the game involve the main character stood in front of them, but not using them. It’s used as a symbol of evil mostly.
In Act 3 there are clerics of a goddess of the sea named Umberlee
One of her clerics drowns and they’re laying her body to rest - the clerics are obsessed with the idea of drowning and celebrate “the lungs filing with water” as as being embraced by their goddess
