There are enemies you can fight throughout the game that either are or clearly resemble real-world animals such as boars, rats, snakes, etc. The game has fairly low graphic detail and most of the animals look very clearly "monstrous" (i.e. spiky and big and menacing) rather than cute but they're present in most of the game and killing said creatures as part of questing or leveling is unavoidable to progress the game.

Grim Dawn is gruesomely violent and gory. As mentioned, some enemies explode into a red mist when hit or killed. But there is a lot of scenery gore as well: dismembered and disemboweled bodies, bloody torture devices and the remains of human sacrifices.

One of the main enemies in the game is an evil cult that drains people of their blood as a sacrifice to the god they worship. In addition, many foes are abominations obviously assembled from human bodies and flesh.

Grim Dawn's graphics quality isn't as detailed as some other games but if blood and gore is an issue, this game is best avoided.
