The protagonist is raised in an orphanage. The yakuza member who runs the orphanage acts as an adoptive father to him and his best friend. It is implied that this man neglected the friend, causing him to grow bitter and jealous.
No but at one point the protagonist fights a trans man, then on discovering that the trans man's friend is a trans woman, fights her. They were both trying to mug him, but still...
A female ally of the protagonist is found bloodily murdered by the antagonists.
Another woman dies at the end of the game, but her injuries are not depicted gruesomely.
An unidentified woman is tortured to death offscreen. We see a notable tattoo on her collarbone up close, but we don't see her face or any of the injuries inflicted upon her.
In a minigame called "MesuKing", sexy women dressed as bugs wrestle one another, but it is not gruesome. Also, this is a videogame within the world of Yakuza, so it's not "real" in the way that the events of the story are supposed to be.
A puppy is shown having rocks thrown at it by young louts. The player character beats up the louts and rescues the puppy. At the end of the game the puppy is abandoned by the player character, although he does find it again in the sequel and take it to an orphanage to be cared for by the kids.
There's a side activity called MesuKing. It's a card game where women in sexy bug costumes fight. At the end of each match the camera pulls back to show beetles watching the match.
A crime lord kidnaps and attempts to rape a woman, but he is shot dead by her male friend before he can carry out the act. All of this takes place offscreen, with the protagonist arriving in the aftermath. The woman suffers trauma and temporary amnesia as a result.

During combat, the player can sometimes use super moves to stamp on or punch the heads of people lying on the ground, but their heads are never shown to be "squashed" or really damaged, and these moves are never fatal despite their brutality.
In various attacks enemies are implied to lose teeth, especially when you attack using either nails or firecrackers, with said attacks focusing almost entirely on oral trauma. When using pliers you can also pull the teeth of enemies.
Although you can throw people off buildings, nobody in the fights actually DIES, and if there's a cutscene afterwards, they'll reappear on top of the building looking beaten up.
A character falls from a pier into water after being hit by a vehicle, but they don't die.
During combat, the player can wield swords and guns, and perform brutal and bloody "heat actions" that do enormous damage and usually involve using a weapon or scenery in violent ways. This is usually met with large spurts of blood. Nevertheless, nobody ever dies from combat, even after you've run them through with a sword or shot them repeatedly in the chest.
Several of the characters are orphans whose parents died offscreen prior to the beginning of the game. Parental figures (but not blood relations) die during the course of the game.
A mixed bag. The only police officers featured in this game are allies to the protagonist, who is a yakuza, but they are seen to be working to take down a bigger threat.
Substory 2: An assassin disguises as a stripper in order to approach his target. A character shouts "It's a man!" before the assassin takes off his wig and shoots someone.
Substory 32: A trans woman challenges Kiryu to a fight, while she is not misgendered, it's implied Kiryu is only willing to fight her because she used to be a man.
Substory 35: Kiryu has an encounter with a woman, but fights him upon learning he is a cross-dresser trying to scam him. He is later joined by his sister, also a cross-dresser who Kiryu mistakes as a man
Majima Everywhere: In Club Shine, Majima appears in drag under the name Goromi, in hopes of initiating a fight with Kiryu. The latter however decides to sit down and chat with her, like he would with a regular hostess. They do end up fighting after the session, after Goromi suggests they go on an after-hours date, which Kiryu refuses, telling her "you're not even a girl" after Goromi accuses him of "breaking a girl's heart"
The game is set in a red light district so there's that in the background. There's a gentlemen's club called Asia that is completely optional (though you do have to go there for a substory and a Majima Everywhere mission). If you complete the storylines for the hostesses, you are shown softcore p*rn videos that can be skipped.
An antagonist is hit by a car and plunges into a body of water. There is a quick-time event in which the protagonist will be hit by a car if the player doesn't input the right button-press in time.
The player character and their foes can both wield guns in combat, although the shots are never fatal.
There is an on-rails shooting section in which the player character must fire a pistol out of a car window at enemies chasing in a variety of vehicles.
Several characters are held at gunpoint in the story.
In a cutscene, an enemy opens fire on the protagonist with a machine gun, who saves himself by pulling another man in front of him and using him as a human shield - the man dies.
A small girl is found in a bar full of shooting victims, clutching a gun. The protagonist rescues her.
A major boss battle involves the player character fighting bodyguards armed with pistols.
