A character makes fun of a species for training (sexually attractive) dancers instead of their military while they are being genocided. The character gets immediately told off for this.

The game's enemies, the Reapers, are often announced by very loud buzzing sounds. They are however not used as jump scares and the audio mixing is not aggressive.

As with the previous two games, you potentially have an obsessive fan. It's possible based on decisions in the first game that he won't be in the third.
In the DLC, you're stalked by a mysterious identity thief. But you can take care of that problem. That's the main plot of that DLC

If you fail to feed your fish, they die. Just have to make sure you feed them every time you go back to the ship. You can purchase a device that does it automatically but it's kind of expensive (in game money).

The Citadel DLC can have a female Shepard abuse their authority as Commander to convince a squadmate to sleep with her after an alcohol-heavy party, despite him being very uneased by the situation. Due to the way dialogue choices work in Mass Effect this can be unexpected to the player.

Husk-type enemies are forcibly cyberneticized from their base species, and are often melted into each other like necromancer experiments. One type has a head clearly on the wrong body; one type has a whole nother corpse acting as an arm-mounted cannon, and one type's three bodies welded together into a mortar team.

As with the first game, if you do things just right, you can make one of the main villains commit suicide. But it's unlikely you'll do the correct sequence of things without a guide.
There's also a situation where a teammate will throw herself off a cliff for very understandable reasons.
Another former teammate tries to kill herself so she doesn't have to kill someone else. You can stop her but if you fail to hit the button fast enough, she will do it.
A third teammate can decide to commit suicide if you advise him wrong. He's the last of his kind.
These are all avoidable if you make the right decisions though it's not always obvious what choice you need to make.

Several characters will sacrifice their life in order to accomplish a mission; most of these can be prevented.
Others will try to commit suicide for various reasons, but these can again be prevented by either making the right choices earlier in the game, or picking the correct dialogue options during the cutscene.

Shepard experiences frequent PTSD symptoms such as night terrors following the opening sequence where they fail to save the life of a child. Some of these instances are playable sequences and therefore cannot be skipped.
If the player chooses the "sole survivor" background for Shepard, occasional remarks and hints will come up that they also experience PTSD symptoms from the event in their backstory.

While no actual miscarriage happens during the game, a character makes mention of one in the past. One of the main missions in the game revolves around the Genophage, a plague that causes most Krogan females to be sterile, and for the fertile few to have difficulty bringing a baby to term. The game goes quite in-depth on the subject, so you may wish to tread carefully if this is a sensitive matter.
