The player character can use drugs (called "chems" in-game) on themself or other characters. These drugs are addictive and have a chance to give addiction, causing withdrawal effects when not taken regularly. One character has a heart problem, and using a drug on him will cause him to die from heart attack.
Several quests include kidnapping, including the main quest (the MC's village is raided and its people are kidnapped). There is also a side quest where a girl is kidnapped and sexually assaulted.
Not Exactly autism-specific but there are some dated and uncomfortable depictions of mentally/intellectually disabled people (which, as an autistic person, I found unpleasant. This might not be the same for everyone, obviously)
If the player character is driving their car while entering Broken Hills, they will hit a character named Lumpy. He loses body parts, but does not die.
