There are earthquakes that cause places to collapse in the Barrens and the Refuge. The first one happens in the room the main character is in, so you can see the people affected. A character does die on screen but it's due to other hazards in the place, not the cave-in. The scene in the Refuge is only distant sound but it causes a very emotional reaction on the characters.
