There's a pokemon named Totodile. Its evolution line is based on crocodiles and alligators. However, you cannot get or find it unless you transfer it from previous games.
Kind of. There's a mechanic called "friendship" in which it shows how much of a bound you have with your pokemon. The lower it is, the sadder the pokemon is when its with you. In Verdanturf town, a girl with a Pikachu asks you if she can check how much does your pokemon like you, so you can check if it does or not.
No, but a pokemon's ability to Dive under water for an extended period of time allows you to spend as much time as you want underwater and resurface in certain lit areas. For those with a fear of drowning, it can be a shock to see for the first time, and be anxiety-inducing slowly swimming from one end to another. However, it's a relief to know you don't have to worry about drowning nor your pokemon (You can even bring out a fire type pokemon when you swim through the seagrass, and they are not harmed by the water, assumably fighting in a big air bubble) and simply reminding yourself to breathe and take a break when you need to.
There is a move called "guillotine" which has a chance to instantly knock out a pokemon. The target only faints, so presumably this doesn't involve literal decapitation.
Around route 113 there's a man who will turn your collected ash into different objects. His dialogues contains him huffing/puffing a little, but there's no other indication that he's struggling to breathe.
There aren't ghosts as we think of he, just ghost-type Pokemon, although some bare a certain resemblance to ghosts the way we think of them.
How to avoid ghost types: 1. When Nincada (bug and ground type) evolves into Ninjask (bug and flying type), an extra Pokemon called Shedinja (bug and ghost type) may appear in your party. Solution? Either fill your party before and or put your Pokeballs in the PC.
2. Use repels* in Granite Cave**, Cave of Origin**, Sky Pillar, Victory Road**, Route 121, Route 123, and Mt. Pyre.
*Repels can be bought from Pokemarts for 350 Pokedollars and will only work if the first conscious Pokemon in your party is a higher level than the Pokemon you will be encountering. You can find out if you are a high enough level by looking the place that you are in up on Bulbapedia.
There is a scene in the beginning where someone is getting chased in circles fast by a pokemon, and they urge you to "Quick!" pick a pokemon from their basket to save them! Rest-assured, you can take your time deciding. I once had to step away from the game to go do something during my indecision. I came back and finally decided, and it didn't affect the game whatsoever.
Baby pokemon do exist and they often appear from egg hatchings. Also, in Lavaridge town, if you talk to an old lady, she'll give you an egg where a Wynaut hatches from it.
One of the rivals is chronically ill. This illness is implied to be respiratory. I personally like that there is representation of chronically ill rivals!
Like a commenter said in another trigger, you can go underwater by the move "Dive". However, you can stay in there as much as you want, as neither you or the pokemon you have can actually drown
