During an optional (but likely) segment in the game, the player characters are described as eating enough that they cannot move. It is avoidable, though - simply do not drink from the mountain's water fountains, found in both the normal world and the Dream World.

No drugs are used. However, there are two optional (but likely) segments featuring dreams not within the Dream World that end in a fake Game Over screen. These are caused by consuming a fountain's water. This fountain exists in both the normal world and the Dream World on a mountain, and heals the player characters after the descriptive segments.

Princess Peach is, as usual, kidnapped by Bowser. Also, as part of Antasma's final boss fight, Dreamy Luigi is pulled from Mario and trapped in one of the orbs surrounding Antasma.

The game is themed around dreams and sleeping, so this is kind of a given. As for straight up unconsiousness, two instances come to mind
-Early in the game, Mario, Luigi, and Leach are all rendered unconsious in the dream world thanks to Antasma.
-When reaching the top of Mount Pajamaja, Bowser and Antasma play the Dreambeats loud enough to put the entirety of Pi'llo Island to sleep. The only ones unaffected are Starlow, Antasma and Dreambert (Due to not being "normal creatures," by Dreambert's own description), Bowser (who was covering his ears), and Mario (Plot armor).

Near the end of the game, Antasma (who is described as a bat) is betrayed by Bowser and falls in battle to Mario and Dreamy Luigi. He does not appear again after this, and is presumed dead. Keep in mind, though, that despite his species name, he does not appear significantly bat-like in most forms, and he is more person than animal.

There is a scene that gets close to it, though - a character jumps from a high place, and surrounding characters begin acting as though the character will not survive (You even receive a message in-game acting as though the character has been defeated). Shortly after, though, this is revealed to be a trick, and you fight the character in a boss fight.

As in most games in the Mario and Luigi series, you battle a creature known as a Wiggler as a boss fight. Wiggler is based on a caterpillar, and is made up of cartoonishly round segments, much like a child's drawing of a caterpillar might appear. Wiggler is very large compared to a regular caterpillar - each segment is about the size of the player characters.
