Almost the entire film takes place in a school for mentally ill teens. They seem to be well cared for though

The mentally ill characters are called names. The male lead's mother acts as though her son's mental illness won't negatively impact his life

One of the characters has a recurring nightmare where he executes people he knows with the sharpened hands of a clock. The executions themselves are not shown, but there is blood visible during these sequences
