"Insane in the Membrane" is infamous for depicting the slow and disgusting collapse of Baxter Stockman's clone body in conjunction with his crippling sanity, to the point where the episode was banned from American TV for years.

It’s heavily clearly implied in the second episode that these rat catching robots eat rats to reduce the infestation in New York. Nothing gory or graphic but very clear on what happen to the rats.

Though not shown on screen directly, one of Stockman’s eyes is mutilated by the shredder. We hear his scream as it happens, and he is later shown with scars and an eyepatch.

The final season's ongoing plot involves saving Splinter after he sacrifices himself to protect the Turtles from a villain from the previous season. However, he isn't dead in the traditional sense, rather he is broken into bits of data that the Turtles have to journey into cyberspace to collect.

Many instances, though most are off-screen. Michelangelo appears to suffer from motion sickness, and is usually the culprit. The episode "The Freaks Come Out At Night" features on-screen vomiting, with cartoony purple vomit being spewed onto another character.
