Spoiler for the ending cutscene of the "Art Therapy" co-op campaign, which is chronologically the last bit of content in Portal 2.

GLaDOS sees that the villain the players have been chasing throughout the campaign is, in fact, a bird who has left a nest with three eggs. Atlas and P-Body (the player characters) quickly scare off the bird. She tells them to destroy the eggs, but then tells them to stop before they do. She is then shown watching over three newly hatched baby birds in an incubator and dotingly says she will raise them to be "killing machines".

Yes, it’s debated that Caroline didn't want to become GLaDOS and that it was pretty painful. Also Wheatley is in pain during his transformation into GLaDOS. Chell hasn’t eaten a lot of food and is put through tests but she’s not exactly “tortured”.

The player character is repeatedly insulted for her weight and called fat, despite not being even slightly overweight. While there is no indication this actually bothers her, and the comedy is mostly derived from how cheap of a bully the offending party is, it might be triggering to players who struggle with body dysmorphia.

Several.

At the beginning of test chamber 7 in Chapter 2, GLaDOS will play a train horn sound over the PA system to try to startle the player.

At the beginning of Chapter 6, the player must solve a short puzzle in order to open the giant vault door to the sealed-off Old Aperture facility. Upon completing this puzzle, a loud klaxon will sound repeatedly until the door is fully open (this takes ~30 seconds).

Just before the first test chamber in Chapter 6, there is a short (~5 second) cutscene where a bunch of lights turn on. There's a loud *thunk* as the power kicks on, and the lights briefly turn the whole screen white.

In audio recordings you’ll hear that many of Aperture’s tests subjects during its development were homeless people.

One character is stranded in space, regretful of their actions. Another character is implied to have been uploaded into a computer. We listen to a third character’s health fail, and it’s implied that they die from their illness.

There is a scene in which the player travels in a large, decaying metal cube that smashes violently into walls. No one is injured as a result.

In the first game, blood will be put behind you on a wall if you get shot by a turret. This is removed in the second game.

No actual guns are used, however you WILL get shot at by robots repeatedly throughout the game.

If you don’t disconnect Wheatley in time, a nuclear explosion destroys the entire facility. It is a huge facility which travel miles underground, and Announcer makes reference to a reactor core, so it’s presumably nuclear or on a nuclear scale.
