Batman’s every movement is followed by one character or another throughout the game. In Victor Zsasz’s interview tapes, it is heavily implied he stalks a woman before killing her. This is optional content. Optional content in the second game reveals Zsasz was apprehended by Batman before he could hurt the woman.

Joker off-handedly says that “slapping Harley around” is “his hobby”. The verity of this statement is unknown as he is never shown physically hurting her and nothing Harley says or does suggests he does.

the whole thing takes place in an asylum, so all of the patients are kind of restrained. but also, both batman and joker are shown strapped down completely at various points, bane is hooked up into a machine and trapped, and some inmates wear straight jackets

Inmates injected with the Titan chemical undergo unpleasant transformation. Their bodies bulge and become larger and freakishly muscled. The same thing happens with Bane during that boss fight. He basically takes super steroids and gets swole in the most grotesque way possible.

Unfortunately most of the antagonists are mentally ill to some degree or another. Easily the worst offenders are the Lunatic enemies due to being more stereotypical than normal (even if you want to argue that it’s partially due to being experimented on into insanity)
