One major character is heavily implied to have a verbally abusive abusive mother. The mother is not present in the game and abuse against this character is never directly depicted.
While no dragons die there is a task where you remove crystals from dragons' head and body using your pickaxe, the dragon is unharmed by this and seems to appreciate it but if you would be triggered by something like this you should know, this is an unavoidable task in the game
No physical violence occurs, but other Spirits on your ship may feel bullied by Mickey, which lowers their mood.
One of Mickey's quests requires the player to intimidate a worker.
Not sure if it counts, but one reptilian Spirit falls into a hibernation-like sleep she cannot wake from in her storyline, and you have to install a heating system in her house for her to wake up. Other Spirits show concern for her well-being until she wakes.
There is a child character in the game who once you complete all his tasks, asks you to take him to the Everdoor (the place where people pass on) he seems to have some understanding of what this means when he asks you to do this for him.
The game centers heavily on death, and a large number of characters are already dead and on their way to the afterlife. You are the one tasked with escorting them to the afterlife. The game goes out of its way to use its design to make you feel the weight of their death and to feel their absence after they leave for the afterlife.
The characters Stella meets were all humans in life, but they have anthropomorphic animal forms as spirits. When they’re in these forms, they’re presumably already dead but haven’t passed over yet
There is a character who has repeatedly cheated on his wife in the past and during the events of the game you go on a quest for him to get chocolates and flowers, he tells you these are a thank-you-for-forgiving-me gift for his wife but instead you find out that he is giving them to a woman he is cheating with. He also repeatedly flirts with passengers aboard your boat.
Well, all protagonists are dead by nature. And the sould that aren't ready to go with the protagonist yet wear cowls in multiple colours. They are all cute and harmless though.
A very large percentage of the gameplay takes place sailing on an ocean on a boat. There is no circumstance where the boat is in any danger of sinking or otherwise in peril. The player character can dive into the water; you dont see anything under the water and there is no chance of the player character failing to resurface or otherwise being in danger.
The Jackie and Daria update released in December 2021 introduced Overbrook, an island with a run-down hospital. The storyline includes prolonged interactions with a mental institution. Jackie isolates Daria in the psych ward and uses frequent ableist/saneist slurs.

The player had given Daria a musical instrument made of glass in a previous quest, and Jackie implies that Daria used it to hurt him.
Multiple characters are implied to have become spirits by suicide. At one point, a major character takes an unexpected action that is contrary to the players expectations and that could be interpreted as suicide or suicide-adjacent.

Tmk, One Spirit swears quite frequently when you talk to them (using words such as 'sh*t', 'cr*p', etc.). Some other Spirits swear on occasion, but not as often.
In the Jackie and Daria update, released December 2021, Jackie uses frequent ableist slurs. He refers to Daria as a “psycho nutcase” and “lunatic”. He refers to the hospital patients collectively as “idiots” and fakers, plus statements such as: “keeping us safe from these morons and keeping them safe from themselves”. There is also dialogue about Daria being a “difficult patient”. As the player, you have to fulfill Jackie’s demands to progress the quest, and he sends you messages even when you are away from the island.
(SPOILERS) Two spirits, both elderly women, develop memory loss during the course of their storylines in a way that resembles Alzheimer's/dementia. The first asks you where her husband and daughter are, and you can only lead her to the Everdoor by pretending to be her daughter. She recognizes you for a split second before referring to you as her daughter again. The second constantly repeats the same questions every day and forgets what she's saying mid-sentence, before eventually telling you she can't go on and asks you to cross her over. She stumbles her sentences during the trip to the Everdoor
The character Daria reveals through Poetic language that she spent many years living on the street prior to her admission to the psychiatric ward. She is also abused by her orderly.
