While they are a robot and not a person, the Mangle, as their name suggests, is a broken animatronic with limbs in all the wrong places. The Withered animatronic could also be considered this, particularly Withered Bonnie who is missing his face and his left arm, and Withered Chica, whose mouth is stuck open and is missing her hands.
