The conceit of this game is that the main character is a boy who kisses various people to strengthen his heart, and many of them are adults. These kisses are NOT romantic or sexual in nature, however; the creator of this game was inspired by the phenomenon of kissing as a casual greeting in some European countries such as France. A few characters react flusteredly after being kissed, but it is meant to be as neutral as a hug.

Not exactly, but you can peek into the homes of underground residents while they are going about their business. They are largely unaware and seemingly unbothered by this, aside from Dr. Fill, who asks the protag not to bother them while they're working on their invention.

In the very beginning of the game, when walking near Julie's bar, Julie and Goro's introductory cutscene involves Julie slapping Goro. Later, upon getting the wine coupon from Bell and bringing it to Julie, Julie and Goro will argue, leading to him throwing a wine bottle at her head.

The main character himself is a child and does get verbally and physically hurt pretty frequently depending on the situation; even if it's meant to be comedic and depicted in a slapstick way, and the character does not appear to be super phased by it, it may still be upsetting. The protagonist can be shot by the police, strangled by a security guard, or kicked in the face, among other things; this causes an animation of the character falling on their back with X's for eyes, usually getting up shortly afterwards. On the game over screen, the character gets stepped on or kicked while down. These scenes are not exactly *graphic* (with a few exceptions -- the strangling scene has a Bad Noise in it and it's possible to be stabbed with an oversized syringe and have the blood drained from the character's body) but still worth warning about.

Two major characters smoke cigarettes, and it might not count but the merchant consistently warns the protagonist that "you can't smoke unless you're 18". It's also implied in some optional dialogue that Funny Cola has effects similar to stimulants and are "bad for you" (cause damage when you drink it) but it's not explicitly stated to be a drug thing?

One character very clearly has a drinking problem, and another exhibits something KIND OF similar to withdrawal symptoms but it's in regards to... pickled vegetables.

A major character is almost always drunk, and his family members frequently talk about how it's ruined his and their lives. It's taken pretty seriously.

Dead frogs and cicadas are found in trash cans necessary for progression, but you do not see the moment of their death. They're portrayed in a low-poly art style with cartoony X's for eyes. In the end-game [spoilers] large anthropomorphic chickens hold a trial and accuse a character of killing and cooking chickens; it's not discussed in detail, and it seems their main problem is actually just that the character is bad at cooking grilled chicken.

Very early in the game, a character chases a dog for stealing his stationary set; when speaking to him, he calls the dog stupid and mentions wanting to "punish" it, but doesn't seem very serious about it. The dog is ultimately unharmed and not distressed.
Also, dead frogs and cicadas may be found in the trash; it is required for one of the story kisses to find a specific one. The protagonist can eat them (and lose health) or throw them away.

An optional character to interact with is a large spider with a marking resembling a kissing face, that the character is intended to kiss. It's not necessary for story progression, and you can avoid it by not being in a specific area of Scarecrow Field (near a tree) between 5:00 and 8:00 pm in-game-time.

Dead cicadas can be found in the trash cans at random, searching for them is necessary for one major character's kiss. They are portrayed in a low-poly, simple art style. One area has the sound of buzzing flies (a manure pit) when you come near it; it is optional. There is also a large spider in one area, about the size of the protagonist, but it is not necessary to see it for story progression.

An optional underground resident in the hospital is chained up. The main character and a notable NPC get handcuffed and later cuffed to a wall as part of a story beat. The main character gets freed and after solving a puzzle, the notable NPC does as well.

The most relevant answer is in regards to the part where the protagonist and another major character are put in a jail cell deep underground in a graveyard, left to die; the burying is not shown. The less relevant answer is the fact that there are many optional characters living underground, but it's by choice and it's pretty spacious -- they do eventually come up from under the ground though.

One optional character is a boxer who is missing a tooth, and this isn't necessarily damage but there is a short quest later in the game where a character's dentures go missing and you need to retrieve them. The model for the dentures is low-poly but still skeeves me out as someone with a phobia of teeth.

There is a point where the protagonist and another character are handcuffed to a cross-shaped slab in a jail cell presumably left to die. There is also an optional character in a gimp suit who is always bound and hanging by shackles from the ceiling, but... it seems self-inflicted.

It's possible to have the main character fall from a great height and get a game over due to taking too much damage, but it doesn't really mean death and it's not portrayed very realistically.

In an optional sidequest, a certain zombie character's eyeballs seem a bit loose like an old doll's, and eventually after a certain amount of meetings, one falls out with a splat noise; the protagonist has to pick it up to continue with the quest. The eyeball isn't exactly realistically modeled, but it is a bit creepy.

It's a little up to interpretation whether a game over really means "death" but a kid sure can get hurt a lot and collapse! It's portrayed in a cartoony way, but the character can get shot, hit, fall from great heights, etc. Aside from that, there is a zombie character who died in an accident while they were a schoolgirl, but meeting them is optional and the death itself isn't shown, just discussed.

This game is all about the protagonist kissing all kinds of people, including those in married or otherwise established couples, but the kisses are not meant to be interpreted as romantic -- it's platonic, and was inspired by how the Japanese developer was baffled to see people kiss in public simply as a greeting in France. Nobody gets upset about the protagonist kissing so many people, and it's actually encouraged.

The main character must take a bath in the hot springs to progress with the story. The boy wears a speedo in localized versions of the game, but is fully naked in the Japanese version. It's not meant to be sexy or creepy or anything, and nobody seems to be watching besides the player, but the manager of the hot springs does compliment (or insult) the boy on his bathing skills when he's done.

At night, a doctor will wander the streets of Long Life Town with a very large syringe. When he sees the player, he will ask for blood donations. Upon answering yes, he will stab the player with his syringe, take their blood, and then consume it. Upon answering no, he will ask their blood type, to which he can either respond by leaving, or taking their blood anyways.
