1. At least at like half time of the book half the characters know something horrible is coming for them and they cant do anything about it
2. The inherent psychological torture of physical things covered in other questions (rape, forceful amputation)
3. The fact that these awful things are done to them by their peers, even friends, and announced beforehand
4. Peer pressure to do horrible things and peer pressure into accepting (not fighting) that the others do horrible things to them
5. Forced to give up concepts like their faith, their hopes, dreams, and perceived parts of their personality

Throughout the book a little (a cut off finger, blood 'proving' lost virginity) but (spoiler) mostly at the end where the main characters form a angry mob and kill a boy, which is described quite gory. They dont have weapons, so they rip him apart by hand. Thats also why I said yes to the broken bones and eye injury questions, I dont think they leave any part of him intact.
