At the end of the game, if you speak to Agent Forsythe about the main antagonist she will let slip that she's considering giving them to Sasha Nein for experiments. When Raz calls her on how horrible this is, she says she never said "experiments," and that she said "therapy." When Raz tries to call her on it again she shuts him down saying "No one likes an argumentative pre-teen, Razputin."

The very opening level of the game is HEAVILY filled with imagery of teeth and other dental bits. I don't know if a tooth gets strictly broken, but it wouldn't surprise me if one does.
It happens prior to the events of the game, but the events of the game pick up just after the Psychonauts rescue an important individual who's been kidnapped. This is depicted in a very cartoony way in the recap of the previous games.
In some variant dialogue at the Motherlobe you can hear two coworkers mention that a husband deserves a surprise party, especially since he was so understanding of the two coworkers' affair.
Spoilerish: At the end of the Compton cooking show level, the boss battle features vomit as the primary attack of the boss. Unfortunately there's no way to avoid it as you cannot progress the story without finishing the boss battle.
The visuals aren't particularly realistic, however I've noticed a number of streamers find the boss battle to be pretty gross and they don't even seem to be emetophobic. It's hard for me to tell how triggering this boss battle would be to someone sensitive to this subject matter.
In Compton's Cook-Off, the food items you cook with are shown to be sentient. They do not show any pain or fear about the cooking process, and their preparation is not shown in a graphic manner
Most characters are mentally ill or struggle mentally, so it's not stereotyping, but some of them do engage in violence. The extent varies character to character, from slapping or pushing to (in the most extreme case) mass murder.
when you visit otto and use his brain tumbler theres a scene where raz is in a dark room after entering the brain and theres a really bright light flickering and girating. it honestly hurt my eyes and gave me a headache.
While walking into the hospital in Hollis’ mind, there is a brief reference to child birth. This is only auditory but can’t be skipped. It’s only a one liner.
Yes and no? In Hollis' Hot Streak the Maternity Ward is turned into a roulette table with babies in every section of the wheel. When the board lands on a baby, a baby appears, fully swaddled, in a parent's arms.
No. There is only one dialogue instance of a character referring to another character’s great aunt as his girlfriend, but she has no idea the two are related.
Nothing on screen, but some characters talk about finding each other "hot" and "fooling around" places they're not supposed to. Some variant dialogue from two minor characters also mentions an affair but doesn't go into detail.
SPOILERS: A car flies through the air and crashes into the ground when Raz runs to find his family near the end of the game. Nobody appears to be in the car.
In Maligula's vault, you can see her drown an entire crowd of peaceful protesters, as well as someone important to her. A resurrected character mentions that his body is probably somewhere deep in the frozen remnants of Maligula's last flood, but we don't see him drown.
Very minor, but if you use clairvoyance on smaller animals, they will perceive you as a blood thirsty monster. The drawing depicting this features blood around the main character's mouth.
