There's a scene where it might seem that way because a man holding a cat gets dissolved into matrix code along with the cat, but we later see the man alive so it can be assumed the cat is fine too.

Not teeth, but a person does get kicked in the head towards the end of the movie resulting in his jaw being broken and sort of hanging open. He makes some gross noises before it gets fixed with a snap. It was pretty upsetting for me, don't know where else to put this.

Mentioned in the suicide comment section, towards the end of the movie there's a scene where people jump out of very tall buildings to [spoilers] try to hit Neo and Trinity on their bike. [/spoilers] Its a lot of bodies and its pretty gross. I don't recall them looking or focusing on any of the bodies too much, but I was looking away a bit and might have missed something.

[SPOILER] I don't know why people are voting NO on this?

A grown-up Sati talks about how her parents were "purged" from the Matrix, their code erased by the Analyst, and how she grieves them. The Oracle, another parental figure/guardian of hers, is implied to have been purged as well.

When Thomas/Neo starts to see the flaws of the analysts lies and the matrix getting hacked the filming style emulates an anxiety attack (according to personal experience). With the analyst giving him help to stop it after (what do you smell, hear, etc…)

(No spoilers, but slightly more clarification) To say that this movie has a happy ending is frankly an understatement. I would even go as far as to say that it's an even happier ending than the ones that involve cheering and parades because this happiness is shown, not told. Finally just about every character that is remotely sympathetic has a "good" ending as well.

[Spoilers] at the end of the film when Neo and Trinity are in the run via motorcycle there are several cars being shown and people jumping out of buildings and landing in front of moving vehicles (the people are matrix code designed to function as bombs).
So visually and technically yes, but with a bit of a separation from reality without a lot of camera focus as well.

(No, but we see people submerged in bio-gel. They get the oxygen they need, since the gel is oxygenated, but it was a bit triggering for me to see people submerged in liquid for minutes.)
