In the second part of the side quest with the washed-up Korean actor, you take him to a park to sober up. Near the end of the cut scene, when Kasuga tells him to "let [his emotions] out", the actor gets sick. There's no sound, just subtitles, and the animation just looks like an unrealistic water splash from behind as he's facing away from the camera. Really, really mild.

There is a dominatrix-themed female-only "job" (a kind of fighting class) called Night Queen. Characters using the Night Queen job can be dressed in a shiny leather/latex-style fetish suit (although they can also wear their normal clothing) Their attacks involve humiliating the enemy men, and they can suggestively rake their fingers on the chests of other player characters (including female characters) to give them a stat buff.

The player character can "romance" several women, and there is suggestive dialogue, but there is nothing sexual seen onscreen.

Several parts of the game take place in and around red light districts in multiple cities, and sex shops and brothels are present in all. In one city, you can buy weapons and armour from a sex shop; one of the weapons is a comically large (though not shaped like an actual penis) vibrator.

The protagonist was raised in a brothel, and sex workers are seen wearing robes and towels as they care for him as a baby, but there is no nudity.

A character gets a job cleaning out a brothel, and is seen holding a bin full of used tissues.

The violence in the combat section is wildly fantastical and rarely bloody, and the main characters never kill anyone.

However, some cutscenes are deliberately much more realistic. In one, a bound man is shot in the head with a crossbow, with a blood spurt as he is struck. In another, a man has his eyes gouged by another man, and there's a split-second shot of blood running out of the eye sockets. There are other cutscene instances of blood/gore, but the latter is the most graphic.

A couple of cut scenes involve gun violence, one of the major characters uses guns as a weapon (can be switched/not used) and several enemies and allies use guns as their weapons of choice.
