One of the main antagonists, a man named Tanner, will use a syringe on you if he breaks into your house and finds you. On the title screen, he can flaunt the needle and flick the tip of it with his fingers. If he gets you in-game, you see him aim the needle behind the viewer's view window (presumably into the player's body) and the screen gets blurry.
