Specifically in the "A Woman's Lot" DLC the main character's dog, Tinker, is killed by a man with an axe in the last scene of the DLC and cannot be avoided. The death happens off-screen but you do see Tinker's deceased body (with no gore other than blood).
In the quest where you raid Pribyslavitz, after the fight with Runt (bald guy who took your sword), Henry vomits while everyone else is talking. I muted the game and covered everything but the captions so I could still get any story dialogue. AFTER Radzig says "That's the fear leaving you, let it out", it's safe to watch.
Yes, there is an attempted assault on Theresa in her flashback.
In a quest in Uzhitz you (and Henry) are not given the chance to consent to a sexual encounter with a local barmaid. You are too intoxicated to give informed consent and you appear to be unconscious for the first half of the... act.
The protagonist becomes unconscious a few times in the beginning of the game. Non-player characters can also be strangled unconscious in their sleep at the players discretion.
