In the closing scene, if Pamela is among the survivors, she will attack with a syringe. She will either use it to revert the rat to the man or be unable to follow through, but if Four and Pamela are the only survivors, the syringe hits Four instead, and the results are extremely gory.

Excluding the prisoners, it is implied that the doctors' families are being held hostage and will be killed if they doctors do not complete their work.
