The antagonist and other enemies can appear out of nowhere in a scary way. This is pretty minor in the case of jumpscares, but it is startling. There's no image flashing onscreen with a screamer, just an enemy occasionally jumping out and sprinting your way when you least expect it.

In the closing scene, if Pamela is among the survivors, she will attack with a syringe. She will either use it to revert the rat to the man or be unable to follow through, but if Four and Pamela are the only survivors, the syringe hits Four instead, and the results are extremely gory.
