The prisoners are confirmed to all be test subjects for the government. You watch two of them meet their end in the opening scene through this method. The antagonist also does this to pretty much everybody.

The antagonist and other enemies can appear out of nowhere in a scary way. This is pretty minor in the case of jumpscares, but it is startling. There's no image flashing onscreen with a screamer, just an enemy occasionally jumping out and sprinting your way when you least expect it.

The canon ending where Four, Six, and Pamela live is a very happy ending. If you do not save them, the ending will be much sadder depending on who all died.

There is a scene where somebody can get crushed alive if you do not save them. Also, in an ending where you do not save everyone, a dismembered head creature can get crushed to death.

There are windows in the facility, and there is a thunderstorm outside. On occasion, the windows will flash white, but not in an epilepsy trigger warning sort of way. That's about all there is though, thankfully.

The opening scene has 3 people being injected with unknown substances, killing two of them and morphing one of them into a gigantic rat person. In the closing scene, if Pamela is among the survivors, she will attack with a syringe. She will either use it to revert the rat to the man or be unable to follow through, but if Four and Pamela are the only survivors, the syringe hits Four instead, and the results are extremely gory.

In the opening scene, one character is a human morphed into a gigantic, murderous rat man and another character's head detaches from his body as he becomes a dismembered head with spider legs. Later, scientist morphs into a violent monster, but you hear him warn the player to run away from him before morphing and clearly does not want to become what he morphs into. All three of these characters are recurring characters who show up in various parts of the game.

Several deaths most likely end in this, but this game has Prince of Persia-style graphics, and as a result you don't really see any bones breaking. The SFX also lack any distinct sound of a bone breaking.

Arguably, though pretty much everybody in the facility is there against their will. Excluding the prisoners, it is implied that the doctors' families are being held hostage and will be killed if they doctors do not complete their work. You don't actually see any kidnapping happening aside from guards taking prisoners back to their cells.
