I find it amazing that this question rarely has a positive answer, and, when it does happen, people answer “no”. Yes, a dragon dies in the Piccolo Daimaoh arc, but is resurrected afterwards.

SPOILER BELOW about the identity of the dragon and when he is killed (take note that the other comment below mine also delivers the spoiler about the identity of the dragon, but without warning)

That dragon is shenron. He’s killed right after Piccolo Daimaoh gets his wish of rejuvenation.

SPOILERS about the family members that die below.

Goku's adoptive grandfather dies offscreen. He isn’t resurrected at any point.

There are two characters, clearly inspired in north american native indians, that live below Karin's tower, a father and a son. The father is killed, but is resurrected by Shenron afterwards.

Visually, Black individuals are shown pretty often in highly stereotypical ways. They have ridiculously large lips, as in their faces are 50% lip. Chinese background characters also appear as racist caricatures, with tiny slanted eyes and buck teeth.
