The villains run a "mental institution" (which is a cover to imprison people for something else) which is very abusive. The villains use stigma against mental health issues to discredit a main character and to continue imprisoning people. One of the main characters is dismissive and rude toward an elderly woman who ostensibly has dementia. One of the main characters is often viewed as "crazy," and she doubts her own experiences/asks people if she is "crazy." I think all of this is depicted negatively.
