On the Outlast wiki, it's revealed that Officer Coyle enjoys sexually assaulting his victims with his shock baton. In-game you also see burn holes in several mannequins, some with burnt holes in their ass. Coyle also seems to place his hand on/near his crotch a lot in a suggestive manner.

There's a grind machine with blood on and around the device, implying people have been crushed in there. In-game a body gets thrown in the grinder to dispose of the body.

When Futterman/Gooseberry attacks a player, she may shove her drill into their mouth; several of her voice lines are related to dental health and drilling into teeth

The player characters are tortured and forced to go into lethal trials. Depictions of torture through mannequins are plenty. Antagonists torture other NPCs and the players.

Players can die, the Prime Assets are also seen killing people, and objectives in a couple of trials require the player(s) to kill an NPC. (Kill the Snitch, Cleanse the Orphans)

Mother Gooseberry's trial is set in an amusement park. There are many mannequins and dolls present throughout the map. One task requires the player to push a rollercoaster cart with children mannequins to a certain destination.
There are also a few animatronics present in the level. Some of the mannequins are mutilated and bloodied.

when playing any trial with franco barbi (bambino, lupara) there’s a cutscene after you enter the shuttle where a mannequin is “burping” him and he gets sick on your character. it’s quick but it’s extremely graphic and very upsetting. i advise anyone with emetophobia to look away for those cutscenes before the actual trial starts. there’s also multiple spills on the floor throughout the trials which are unspecified but some of them look a bit like sick. overall a very disgusting game but APART from that it’s definitely worth a play

No, but in one Trial, you must electrocute an NPC. In the same trial, there's an enemy with a shock baton which he uses to electrify other NPCs and the player(s).

The Futterman puppet often makes misogynistic remarks.

A live man in a mask is displayed, with degrading remarks carved in his skin and several bite marks on him, appearing to have been sexually assaulted. The whole process of this trial is to humiliate and objectify this man, then incinerate him.
Coyle also makes many objectifying comments to female Reagents.
Mother gooseberry is sexually objectified by both of the other prime assets in the prime time settings.

It's an outlast game. Be prepared for a lot of gorey and bloody content. Mutilated corpses, large pools and trails of blood are already visible in the released content so far.
