we never see real living children in the facility but abuse of children in a cult is mentioned and many trials are themed around abusing kids (altho they are clearly mannequins, an antagonist genuinely believes they are children and tries to stop you bc of this)
both women and men get brutalized for spectacle, with men being the majority of the victims, and most of the sexualized/rapey kills have male victims. avoid trials with the prime asset franco if you dont want to see brutalized women, those maps have the most dead women in the game.
most of the antagonist characters believe the trial environments are real and this is a reinforced belief by murkoff, so kinda. its not mentioned much tho if you aren’t paying close attention to prime assets

Documents related to Futterman mention she was imprisoned for using her TV show as a front for getting children addicted to drugs. During the Cleanse the Orphans trial, a film reel of her singing a song about her cocaine usage can be heard.
not technically, as in you dont see penetrative rape or any sex on screen, but theres frequent graphic sexual harassment in the game, people get sexually humiliated and stripped or even mutilated, and rape threats towards the player are common
On the Outlast wiki, it's revealed that Officer Coyle enjoys sexually assaulting his victims with his shock baton. In-game you also see burn holes in several mannequins, some with burnt holes in their ass. Coyle also seems to place his hand on/near his crotch a lot in a suggestive manner.

Mother Gooseberry's trial is set in an amusement park. There are many mannequins and dolls present throughout the map. One task requires the player to push a rollercoaster cart with children mannequins to a certain destination. There are also a few animatronics present in the level. Some of the mannequins are mutilated and bloodied.
when playing any trial with franco barbi (bambino, lupara) there’s a cutscene after you enter the shuttle where a mannequin is “burping” him and he gets sick on your character. it’s quick but it’s extremely graphic and very upsetting. i advise anyone with emetophobia to look away for those cutscenes before the actual trial starts. there’s also multiple spills on the floor throughout the trials which are unspecified but some of them look a bit like sick. overall a very disgusting game but APART from that it’s definitely worth a play
kinda? the sinyala facility definitely has a focus on psychology, and the trials are called “therapy” by the staff and the player is repeatedly told they are being helped and made better and its got a medical theme to it, but its not actually with the goal of helping any of the patients, it is a psychological torture facility pumping out amoral killing sleeper agents.
No, but in one Trial, you must electrocute an NPC. In the same trial, there's an enemy with a shock baton which he uses to electrify other NPCs and the player(s).

every solo rebirth ending ends up with your reborn reagent dead once they fulfilled their purpose. communal rebirths are not as detailed and its implied the players are on the run but theres no way to know
It's an outlast game. Be prepared for a lot of gorey and bloody content. Mutilated corpses, large pools and trails of blood are already visible in the released content so far.
