You can rescue an NPC who is in a "relationship" that is dubious at best. Probably there has been sexual assault, but this is not explicitly discussed. The victim seems to care for the abuser.
There are tons of caves and such, but the top-down view of the game means you always have a wide, clear view. It might seem claustrophobic to the characters, but not the player.
No animals are specifically abused, but there are lots of animal monsters which you have to fight, and some harmless animals in the background which you CAN hurt/kill.
Not explicitly torture purely for the sake of extracting information or causing pain, but a number of NPCs are enslaved and treated horribly. Most of them you can end up rescuing.
No, but FYI there is a plot point in the Heart of Winter expansion involving a dragon making changes to her offspring. She doesn't kill them, but it's not pleasant.
It would be really gross if the animations were much bigger and much more realistic (sprays of blood are literally just red pixels). Also, you can turn it off.
There are some NPCs (generally ghosts) with memory loss or personality changes, or who don't seem in touch with reality. Some of them can be healed, but not all.
