An instance of graffiti in Oh Shit! includes the text PIZZAFACE EATS PIZZA, with Pizzaface being the main villain and also a giant pizza. While the difference between sentient and non-sentient pizza is not obvious, it could still be classed as a reference to cannibalism (though we do not actually see Pizzaface eat pizza at any point). ...There's also a much grimmer implication in The Pig City, where a hidden room seems itself fleshy and is full of bacon pickups--in a level where the populace are mostly swine!

Maybe? Sometimes when you hit/kill enemies, an eyeball can be seen flying, however it isn't gory and is more like cartoony slapstick. Also the enemy doesn't even lose an eye.

The fourth boss is consistently uncanny in motion and in sound. Also, the splash screens for the bosses are overly detailed compared to the rest of the game, and may be uncanny in comparison. ...In addition, for every 10 times in a level that you harm Peppino, a different disturbing image will show up in the TV in the corner, some of which fall under this.

In The Pig City, one of the taxis you take actually turns out to be a police cruiser in disguise, and takes a clearly distressed (well, more distressed than usual, and that's saying something) Peppino to a jail cell without any reason provided.

To elaborate on Don't Make A Sound, if the alert is sounded and an animatronic catches up to Peppino, each one will provide a unique jumpscare (uncanny image and grating noise) before resetting his position to the start of the screen...except that the more you get jumpscared, the higher the chance it will instead play a yodeling noise and show the animatronics celebrating Oktoberfest with Peppino (though still resetting him). If this troubles you, there is a mod available that can always make Oktoberfest kick in.

Kinda, the level gimmick of Pizzascare has a ghost possess various objects to harm Peppino, I don't think any of these are sentient however.

In the second boss, a ghost may show up and present a source of harm, though for some convenient reason you can shoot the ghost to dispel it. In The Wasteyard, a ghostly mushroom can turn Peppino into a ghost (though this is easily reversed, and returning to normal causes a white sheet to fall away), and the chase sequence here is coupled with a ghostly Pillar John stalking you through every segment and teleporting you to the start of the screen if he catches up. Pizzascare features ghosts as foes that cannot be conventionally defeated, though a priest can help you dispel them. And since the final level takes a bit from every level before, aforementioned gimmicks show up again, though you will also be able to shoot some.

In The Pig City, most of the citizenry are swine. This also happens to be the only level where police officers are shown, and they're shown to be more of a hindrance than anything--handcuffing Peppino (and Brick, later) and even pretending to be a taxi so he'll get into the cruiser without having any clear reason. (The toppings are just lying around, surely that's not stealing?...) However, at the final judgement at the end of the game, one of the judgements you can receive is "That's the one, Officer!", which is an upset Peppino pointing at the screen with one of the pig cops in the background following his motion; make of that what you will.

Some levels, most notably Don't Make A Sound, have red splatters around. Given the thesis of the game, it's PROBABLY tomato sauce, but in many cases it certainly invokes blood. Most notably, the sausage butcher in Don't Make A Sound has a quite bloody visage if it catches you.

Some enemies in Fastfood Saloon use guns against Peppino, though like in any other level, he can't be killed by them. The second boss both uses a revolver and provides one to Peppino to even the playing field. The third boss pulls out a minigun at the very end, but doesn't get to use it. In Don't Make A Sound, a shotgun is provided to Peppino at the level's turning point, though it will mostly be used on mechanical entities. In WAR, a shotgun is provided at the very start of the level, and enemies have various arms. In the final boss, a revolver is again provided, the boss themselves uses an Uzi-like that shoots cheese and dough on THEMSELF, and the second boss shows up again. And in the final escape sequence, the shotgun sees some more use towards the start, though it is eventually confiscated. In all of these instances, while the shotgun can instantly take out tanks, animatronics, and somehow ghosts, the usage is far from detailed.
